Looks like damage from a hand-truck or dolly used for deliveries.

I sure hope no people are delivering up and down 2 stories at a parking garage.

Thank you for reporting! The City is aware of the issue on the Grand Staircase and have re-secured the loose thick paver slabs. The City has received the special order tiles, will replace the appropriate tiles, and fill in the gaps once we have some dry weather to allow the work to dry properly.
